Membership Agreement
2011 - 2012 School Year
I agree to the following:
1. Payment of the $45 (new students) or $35 (returning students/alumni) non-refundable Registration Fee when submitting my complete Registration Form.
2. Payment of the first month’s tuition (September) is due May 1st, prior to the new school year and is non-refundable. Tuition for the school year shall be _$95 per month for the Stars (3s) class; $145 per month for the Rockets / Pre-K (4s & 5s) class. Special fees (includes insurance, PCPO dues, and a project fee) of $91.50 are due by June1st and guarantees a place in the class. Non-payment will cause students to be placed on a waiting list and their place in class may be lost.
3. Payment of last month’s tuition (May) is due by the first day of school.
4. Payment of monthly tuition is due by the first day of each month and it will be delinquent if not paid by the 10th. A late fee of $20.00 will be applied if tuition is paid after the 10th.
5. Return all completed forms, medical record forms, and immunization records by the first day of school.
6. To keep my child at home if there is any sign of a cold or communicable illness.
7. To assist as one of the two required parent helpers in the classroom as scheduled or to exchange with another parent if I cannot help on my scheduled day.
8. To provide a nutritious snack on my assigned snack days.
9. To provide transportation for class field trips when needed and to make sure that all children riding in my car are properly restrained in accordance with State Highway Regulations.
10. To attend the four MANDATORY General Meetings scheduled for each school year. These meetings will be a General Meeting in August, an In-Classroom Orientation prior to the first day of school, a Parent Education Meeting in January and the Spring General Meeting in March/April. I understand that although both parents are encouraged to attend such meetings, either parent may attend to represent the family. Please note that children are NOT allowed at these meetings.
11. To participate in one of three work parties scheduled during the school year. These work parties are scheduled for one prior to school opening, one at mid-year in January, and one at the end of the school year.
12. To participate in maintenance of the schoolrooms and grounds as regularly scheduled by the maintenance chairperson.
13. To participate in all MANDATORY fundraising activities (minimum 2 per year) for the good of the school. Profit and/or Contribution to be determined yearly by the Board.
14. To hold either a position on the Board of Directors or a Committee Position and to carry out all duties necessary to fulfill the obligations of that position.
15. All Parents and other volunteers who assist in the Preschool or on field trips for more than four hours a year must complete and return the Oregon Dept. of Education form called the “Criminal History Verification of Applicants.” This form is provided with registration materials.
